(7) AFFIDAVIT MUST BE NOTARIZED BY BANK AND SCANNED IN

(8) SUBMIT!!! SUBMIT!!! SUBMIT!!! THE MORE YOU SUBMIT THE GREATER THE NUMBER OF APPROVALS DEADLINE FOR SUBMISSION IS THURSDAY @ 10:00AM. APPLICATION MUST BE SUBMITTED ALL IN ONE EMAIL. WE WILL NOT EXCEPT APPLICATION IN MULTIPLE EMAILS.
